This interdisciplinary team consisting of a physician, rehab nurse, occupational therapist, physical therapist, speech-language pathologist, psychologists, neuropsychologist, case manager, and dietician are available to address a multitude of special needs that impact patients and their loved ones after a stroke. The team will meet with you regularly to discuss your progress, update the rehabilitation plan, prepare for your discharge home and make recommendations and referrals for any continued services that may be necessary.
