Catholic Health Services offers both long-term and short-term skilled nursing services. Long-term care residents benefit from a fully integrated and interdisciplinary team approach to care. Specialists on staff include Medical Directors, Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, Dietician, Activities Coordinator, Social Worker, Case Managers, Occupational, Physical and Speech Therapists.
CHS facilities offer a high nurse to patient/resident ratio and takes pride in a staff who are professionally trained to manage special needs such as wound care, infusion therapy, pain management and palliative/comfort care. Short-term skilled nursing services are also available to our rehab patients and can include IV therapy, wound care, medically complex care and more.
Skilled Nursing Care
Skilled nursing facilities (SNFs) are for patients whose conditions requires around the clock oversight and treatment that can only be given by licensed nurses. SNFs provide rehabilitation and nursing care for patients who require short-term recovery following an acute illness or accident. Skilled nursing facilities also are for patients who may be bedfast, or who require treatment of chronic conditions as well as assistance with the activities of daily living for extended periods of time.
Sub-acute or Transitional Care
Sub-acute care is post-hospital, often for individuals with complex medical conditions that is more intensive than skilled nursing care, but less intensive than hospital care. Sub-acute care is for patients who continue to need intravenous therapy, tracheostomy care, or treatment of extensive wounds. It is also for patients who require intensive rehabilitation for various neurological and orthopedic conditions.
Our programs are run under the supervision of highly qualified medical directors. The medical directors are in consultation with the patient's primary physician to monitor and implement the specific medical or rehabilitation protocols prescribed for the individual.
