The St. Catherine's South Outpatient Clinic Team works with one important goal in mind; to design a program individualized for each patient that helps them to achieve their maximum level of functional independence.
Success of rehabilitation depends on the expertise of the specialized healthcare professional dedicated to the continued improvement of the patient. The St. Catherine's Rehabilitation Hospital multi-disciplinary team will meet with the patient and family to discuss progress, adjust the rehabilitation plan as needed and prepare discharge plans to a safe environment best suited for the patients needs.
